Our Community Health Team has one mission… to help you improve your overall health and wellbeing.

Our team consists of Nurses, Social Workers, Registered Dieticians, Care Coordinators, a Community Health Coordinator, and a Medication Assistance Program Coordinator. Each team member provides expertise in specialized areas to help you be as healthy as possible. Community Health Team services are located directly in primary care, pediatrics, and women’s health. These services are free for anyone, regardless of income level, which is made possible by participation in the VT Blueprint for Health and OneCare ACO programs.

  • Diabetes education & support
  • Chronic disease management
  • Nutrition counseling & weight loss support
  • Prevention and wellness support including VT self-management workshops on:
  • Diabetes Prevention
  • Diabetes Management
  • Smoking Cessation
  • High Blood Pressure Management
  • Brief counseling interventions and assistance finding long-term therapy if needed
  • Assistance connecting to community resources and agencies
  • Help coordinate your medical care
  • Assistance with medication affordability (based on financial need)
  • Help filling out advanced directives and long-term care applications
